Your First Appointment
PATIENT RECORDS TRANSITION: In May of 2021, Brodie Rice, CPO transitioned from Campbell County Health to his privately owned Prosthetic and Orthotic clinic - Pivot Prosthetics and Orthotics. The team at Pivot will provide continuation of care of previous patients. All services previously offered through the CCH Rehab Orthotic and Prosthetic Department are now being offered at Pivot Prosthetics and Orthotics.
To Transfer your patient record, please call our office at 307.696.8016 to initiate this process. You will need to provide your full name and date of birth. A record request will then be sent to Medical Records at Campbell County Health.
Before you arrive to your first appointment, you can register with all of your information using the form below.
You may also plan to fill out your forms in our office on the day of your appointment. If so please arrive 10-15 minutes early.
Other items to bring to your first appointment:
-1 Form of ID (i.e. Driver’s Lisense)
-Insurance Card
-A prescriptions/physician’s order from your referring or primary care doctor.
